If you need fingerprints for Home Health Care, you will receive a referral form from your employer or agency. At the top of this form, you will have the following code numbers: ORI, DPS Account, Reason, OCA. You must bring this form in with you as we need those code numbers. The FBI will automatically update the NABBS system that your employer searchers to see your results.

IMPORTANT: Home Health Care and Nursing are different license types. Sometimes people use these terms as synonyms. When you come in for fingerprints, please specify you are there for Home Health Care (don’t say “I”m a nurse”) and give us your paperwork. 

The fee is $68 (Includes $40 FBI fee, $5 DPS fee, and sales tax). Pay by cash or card.
